Section F. – HIPPA REVIEW/AUTHORIZATION
I understand that: Once Integrated Orthopedics of Arizona discloses my health information by my request, it cannot guarantee that Recipient will not re-disclose my health information to a third party. The third party may not be required to abide by this Authorization or applicable federal and state laws governing the use and disclosure of my health information. I may make a request in writing at any time to inspect and/or obtain a copy of my health information maintained at this facility as provided in the Federal Privacy Rule 45 CFR (164.524). My records are confidential and cannot be disclosed without my written authorization except when otherwise permitted by law. The medical records to be released may include, but is not limited to: history, diagnosis, and/or treatment of drug or alcohol abuse, mental illness, or communicable diseases. This Authorization will remain in effect for one year or I provide a written notice of revocation to the Medical Record Department (45 CFR 164.508(c)(2)(i)).
THIS AGREEMENT STATES THAT THE RESPONSIBLE PARTY AGREES TO TERMS STATED BELOW,

We will bill primary and secondary insurances. You are responsible for deductible, coinsurance, copays, uncovered service, plus any supplies purchased and not covered by the insurance. If you are covered by Medicare, we will bill Medicare as your primary insurance. We will bill any secondary insurance also. You are responsible for deductible, coinsurance, copays, uncovered service, plus any supplies purchased and not covered by the insurance. You may be asked to sign an ABN (Advanced Benefit Notice) for services that are noncovered by Medicare.

This financial agreement is based on information quoted by your insurance carrier via telephone, because your Insurance carrier may misquote your benefits to us, we strongly encourage all patients to verify their own benefit coverage, including co-pay amounts, remaining deductibles. THIS FINANCIAL AGREEMENT IS BASED ON BENEFITS QUOTED BY YOUR INSURANCE CARRIER AND IS EFFECTIVE THROUGH THE CALENDAR OR FISCAL YEAR, WHICH EVER CORRESPONDS TO YOUR INSURANCE POLICY.

If reimbursement is to be received due to a personal injury, all adjustments are null and void and full balance without negotiation will be due at the time of settlement.

All co-pays and co-insurance payments are due prior to treatment. We accept cash, check, or credit card.

Should you be unable to keep a scheduled appointment, you must call at least 24 hours prior to your appointment. Patient’s, who fail to do so, will be charged a $35 fee. These charges will be the patient’s responsibility as insurance carriers will not pay for them.

Your insurance coverage is an agreement between you (the patient) and your insurance carrier. Integrated Orthopedics will, as a courtesy, submit all eligible charges to your insurance carrier for payment. If Integrated Orthopedics is not contracted with your insurance company, you are personally responsible for all fees incurred by your surgery. As your insurance has a contract with you and not with Integrated Orthopedics, it is your responsibility to seek reimbursement for your procedure from the insurance company. Please remember that you are ultimately financially responsible for all charges incurred during your course of treatment. A statement of charges showing patient responsible charges (those charges that are not covered by your insurance carrier) will be sent out monthly. A patient who has not patient responsible charges will not receive a statement until their course of treatment is completed. Upon completion of treatment, all patient’s will receive a statement showing all pending charges, adjustments and pending insurance payments. Any charges, which are the patient’s responsibility, are due immediately.

Surgery Patients. We expect payment for the estimate of patient due at the pre-operative appointment and payment of the remainder of the fee prior to your surgery. Failure to pay for your fees due prior to surgery may results in your surgery being postponed to a future date. Any additional procedures will be added to the bill at the time of the procedure. If, after 90 days from your discharge date, we have not received payment in full from the Insurance Carrier, all outstanding charges will become the responsibility of the patient and are due immediately. We strongly encourage you to contact your insurance carrier, during this 90-day period, to check on the status of your claims. Please feel free to contact us if your insurance carrier needs additional information from us to process your claims.

I understand that I am financially responsible for all charges incurred. Should this matter be turned over to our collection attorney all costs, including reasonable collection fees (35%-50%) and any court costs incurred by Integrated Orthopedics or our attorneys, shall be the responsibility of the patient or responsible party.

STEROID and HYALURONATE - INJECTIONS
Hyaluronic acid and derivatives (Rx) Brand and Other Names used by Integrated Orthopedics: Orthovisc, and Monovisc Corticosteroids (Rx) Brand and Other Names used by Integrated Orthopedics: Steroids and Kenalog This is a summary and does NOT have all possible information about these products. This information does not assure that these products are safe, effective, or appropriate for you. This information is not individual medical advice and does not substitute for the advice of your health care professional. Always ask your health care professional for complete information about these products and your specific health needs. USES Injections are used to treat pain in patients with joint inflammation. They are usually used in patients who have not responded to other treatments such as acetaminophen, exercise, or physical therapy. Steroids are synthetic drugs that closely resemble cortisol, a hormone that your adrenal glands produce naturally. Cortisteroids are different from the male hormone-related steroid compounds that some athletes use. Steroids work by decreasing inflammation and reducing the activity of the immune system. At IOA, we use steroids to treat a variety of inflammatory diseases and conditions including tendon, joint, and bursar inflammation. They are given systemically, which means throughout the “system” or body, or locally, to the precise place where a problem exists. Hyaluronate (also known as hyaluronan) is similar to a substance that occurs naturally in the joints. It may work by acting as a lubricant and shock absorber in the joint, helping joints to move smoothly, thereby lessening pain. At IOA, we commonly use sodium hyaluronate intra-articular injections to treat pain from osteoarthritis. Hyaluronate injections are typically given in a series of 3 or 4. It is important to have this medication injected on schedule. It may take up to 3 injections before the full benefits of this medication occur. Before Using Steroid or Hyaluronate Injections In deciding to use one of these medicines, the risks of taking the medicine must be weighed against the good it will do. At IOA, this is a decision that we will make together. For these medicines, the following should be considered: Allergies Please tell us if you have ever had any unusual or allergic reaction to this medicine or any other medicines. Also tell us if you have any other types of allergies, such as to foods, dyes, preservatives, or animals including bird proteins, feathers or egg products. These products may contain inactive ingredients (such as latex found in the rubber syringe cap) which can cause allergic reactions or other problems. You should not use injection therapy if you have a bleeding or blood clotting disorder such as hemophilia. It is not known whether these forms of injections will harm an unborn baby. Tell us if you are pregnant or plan to become pregnant while using these medicines. It is not known whether these medications pass into breast milk or if they could harm a nursing baby. Tell your doctor if you are breast-feeding a baby. There isn’t an exact age that demarcates the use of steroids. Hyaluronan is not approved for use by anyone younger than 21 years old. After using Steroid or Hyaluronate Injections For at least 48 hours after your injection: Avoid jogging, strenuous activity, or high-impact sports such as weight lifting and tennis. Do not strain your treated joint for two days after receiving these medicines. Side Effects Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects; a very serious allergic reaction to these drugs are rare. Common side effects may include: • Increased pain 24-48 hours after injection o Ice & Advil if able • joint pain, stiffness, or swelling; • itching, numbness, or tingling; • some headache, dizziness; • back pain; • swelling, pain, redness, or mild discomfort where the medicine was injected. • red flush cheeks • discoloration at injury site Call us at once if you have: • bleeding; • increased pain; • signs of infection (warmth, swelling, redness) around injection site; • a flare or skin reaction; • no improvement in symptoms. Get emergency medical care if you have: • severe headache and/or dizziness • fast/pounding heartbeat; • fever; • tingling skin; • rash, burning or itching/swelling of the skin; • progressed unseen infection; • painful, red, pus filled blisters in hair follicles.
